Care2 <> FRU Account Setup

This form is to be used by the CARE2 team when there is a new CARE2 account that requires FUNDRAISE UP for CARE2 products and campaigns.
This form will be sent to the CARE2-FRU Slack channel as well as sent to the Fundraise Up Partnerships team for processing, CCing Eric Rardin. PLEASE CONFIRM WITH ORGANIZATION IF THEY WOULD LIKE A CARE2 MANAGED ACCOUNT OR A STANDALONE FRU ACCOUNT (preferred). See comparison card
Fundraise Up will: 1. Check if they are an existing FRU customer or have an FRU account and if not... 2. Lead the onboarding component for Fundraise Up, ensuring the nonprofit is up and running correctly, leveraging all best practices and data retrieval for the NPO 3. Communicate back to Care2 via the Slack Channel the ACCOUNT ID needed for Care2 Integration
